Job description
Position: Case Manager
Tenure: 6 Months+ contract
Pay Range: $40.23 to $43.30/hr on W2
Location: Columbus, OH, Hybrid
Job Description:
Position Hybrid, home visits needed to be done
Prefer them to reside in either the Columbus or surrounding counties or the Cincinnati area we do occasionally have in person team
Previous Case management experience needed
Responsibilities included Assessments, Case Management, resource help, care plan creation
RN or LSW licensure is required...
