Job description

Essential Functions of the position are detailed below and include any physical requirements below that. Accomplishes clients' care by providing crisis case management and supportive services to clients and/or their families to promote self-sufficiency; Conducts needs assessment with clients to determine referrals to both internal and external programs; When able, directly connects client to programs; Serves as point of contact for local parishes and priests collaborating with Catholic Charities; Supports Charities Hub Case Managers and Call Specialists with complex client situations Knowledgeable about community resources and research new community resources to fulfill unmet program needs; Advocates on behalf of clients to ensure access to services for which the client is eligible; This position may conduct home visits in determining clients' eligibility Assists Director with quality assurance, operational and administrative tasks; Maintains up-to-date and accurate client files and databases as required by program policy and governing contracts; Responsible for maintaining, updating, and submitting routine and ad-hoc reports This position may require participation in community outreach efforts and must maintain a flexible schedule to attend community events, committee meetings and special events, as needed, and must be willing to travel between sites to fill in for staff May be required to perform other duties and special projects as assigned PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.
