Community Health Worker. Early Childhood Supports and Services

Job description

The Early Childhood Supports & Services program, located in Jefferson Parish, will serve as the local early adopter site for children (0-5) and their families in Jefferson, Orleans, St. Bernard, and Plaquemines Parishes. Community Health Workers report to the Site Director of the Early Childhood Supports and Services (ECSS) office. Community Health Workers (CHWs) are responsible for making contact with families identified as needing assistance from ECSS. CHWs will assist families with identifying support and mental health needs, registering them with the ECSS clinic, supporting families as they obtain needed services, and will be the liaison for the families and their infant and early childhood mental health clinician. CHWs will attend ECSS Network partner meetings and support the families in their interactions with outside agencies, checking in with families regularly to ensure that their needs are being met. CHWs will go into the community to identify families who are eligible for ECSS services, and to meet with ECSS-enrolled families.

Ability to build rapport, work with and advocate for children and families.
Ability to communicate with individuals to share information, plan, and facilitate problem solving.

Ability to form a strong commitment to the Greater New Orleans community and its members.

Ability to work with community members from diverse backgrounds.

Ability to work independently.

Familiar with operation of electronic office machines (phones, copier, fax).

Proficient computer knowledge required; working knowledge of Microsoft Office Software, including Work and Excel; ability to learn electronic medical record documentation.

Ability to participate in a comprehensive, competency-based community health worker training program (LaCHON).

Ability to present in public speaking environments.

High School Diploma or equivalent

One year of experience working or volunteering with community-based programs

Bilingual communication skills.
Experience with young children.

Ability to communicate in a culturally sensitive and trauma informed manner with all families.

Ability to engage and communicate successfully with ECSS team members to best advocate and coordinate services for families....
