Job description
Status: Full Time Hourly FLSA Classification: Non-Exempt
Summary of the Position: The Community Living Specialist is responsible for providing Community Living Support services to Waiver participants and their families to facilitate the Waiver participant's achievement of his/her goals of community inclusion and remaining in the their home.
Qualifications/Requirements:Bachelor's or Master's Degree in Human Services related field as defined by Texas Administrative Code.
Position required reliable transportation, valid driver's license, and current auto insurance coverage.

- Services may be provided in the participant's home and appropriate community setting.
- Deliver skills-based interventions targeted at enabling the waiver participant to attain or maintain his/her maximum potential, while facilitating the Waiver participant's independence and integration into the community.
- Accurate document services, attending Wraparound Team meetings, and communicating participant progress to supervisors, team members, and the referring authority.
- All services are delivered in compliance with the YES Waiver Policy and Procedures manual, YAP Standards, and all state/federal regulations governing the delivery of service.
